When most of an arch is gone, replacing teeth one at a time stops making sense. All-on-4 treats the arch as a whole: four implants are positioned where the bone is strongest, and a single fixed restoration is carried across them.
The angling is the point. By tilting the rear implants, the technique reaches denser bone further forward, which is why it can sometimes be planned for patients who were told they did not have enough bone for a conventional set of implants.

A conventional full denture sits on the gum and relies on suction and shape to stay put. All-on-4 is anchored, which changes how it feels and what you can do with it.

- What does All-on-4 mean?
- It refers to replacing a full arch of teeth using four dental implants. Two are placed toward the front of the arch and two are angled at the back, and a single fixed restoration is carried across all four.
- Is it removable?
- No. The restoration is fixed to the implants, so it is not taken out at night the way a conventional denture is.
- How is it different from a denture?
- A conventional full denture rests on the gum. All-on-4 is anchored into the bone through the implants, so it does not rely on suction to stay in place and does not need to cover the palate.
- Do I need a bone graft first?
- Sometimes, and sometimes not. The angled placement is designed to use the bone that is already there, which is why it can occasionally be planned where straight implants could not. Whether grafting is needed is decided from your imaging at the consultation.
- How many appointments does it take?
- More than one. There is a consultation and planning stage, the surgical appointment where the implants are placed, a healing period, and then the appointments to fit and adjust the final teeth.
- Is it suitable if I am only missing a few teeth?
- Usually not. All-on-4 is planned for extensive tooth loss across a whole arch. For one or a few missing teeth, a crown, a bridge or a partial denture is normally the better option.
- How do I clean it?
- Brushing, cleaning around and underneath the restoration where it meets the gum, and keeping to your hygiene appointments. The gum and bone supporting the implants are what need protecting.
